Apply For A Roverchase Service Dog 

STEP 1

Fill out the application for service dog training and have your medical healthcare provider fill out the required medical paperwork.
STEP 2
Email application and medical paperwork to [email protected].
STEP 3
Only when all forms have been submitted The Roverchase Foundation Admin Team will email you with a Scheduling Link to have your meet-and-greet zoom call with the application team.

Apply for a Roverchase Facility Dog 
Please note that we only place service dogs from the Roverchase Service Dog Program.  If you have a family pet that you would like trained as a Service Dog, we do not offer that service.  If you have questions about this policy or information on how to best meet your needs, please feel free to contact us.
